2E is pretty wide - there is also 'D' for standard 'wide' in-between normal toe-box and 2E. I spent pretty much my entire childhood (other than when playing rugby) barefoot and 2E is plenty for my wide toes (D is pretty tight though).

Pre-kicking tees, when no sand was available, we didn't necessarily just heel a divot and put the end of the ball in. You would usually quickly heel a circle of 4 divots, giving yourself a raised tee of sod to kick off. Was pretty effective.
